Diabetes disease management. Diabetes
is one of the most serious health problems facing our country with prevalence increasing
at epidemic proportions. According to the American
Diabetes Association, 18.2 million
people in the United States have diabetes. Every year, another 1.3 million aged
20 and older – people who make up the American workforce – are diagnosed
with the disease.
Often, people with diabetes (or diabetes mellitus) do not realize the seriousness
of their condition until overt and costly complications present themselves, such
as heart disease, high blood pressure, kidney disease, gangrene, amputation or blindness.

Cardium Health’s disease management program for Type I and Type II diabetes is
people-centered, multi-disciplinary
specialty and integrated to
help people learn to better manage their conditions. Through frequent one-on-one telephonic
counseling with a certified
diabetes educator as a primary health coach and specialty health coaches
as needed, participants learn how to monitor their blood sugar every day and
balance their intake of carbohydrates. They learn how to shop for and prepare balanced
meals. Over time, they develop healthy eating habits to lower their cholesterol and
control their blood sugar levels. They discover exercise programs they enjoy and develop
routines they can live with to achieve successful weight management.
