Evaluating clinical risk, readiness to change and health
literacy. The first call with new participants
is the single most important element in achieving successful health outcomes. That is
why Cardium Health's assessment process takes a three-tiered approach, assessing
not only the clinical risk, but also
the individual’s readiness to change and level
of understanding about his or her conditions. All three components are incorporated into
the Cardium Health proprietary assessment tool. This tool provides an evaluation score
to aid in appropriate program intervention and clinician assignment.
People are more than their medical histories. Many other factors including environmental,
psychosocial, and economic considerations contribute greatly to an individual’s
ability to manage his or her health.
When Cardium Health Senior Health Management Clinicians connect with new participants
for the first time, the goal is not just to ask a series of questions but also to listen
to what the individual has to say. Perhaps a person with diabetes works third shift and
has not been able to eat on a regular schedule as his doctor instructed. Maybe an individual
knows that she needs to lose weight to control her high
blood pressure, but her mother
recently passed away and life is still too stressful. Or sometimes, an individual may
not have understood when his doctor told him his high
cholesterol could lead to a cardiovascular
blockage.
Our assessment process allows us to determine how we can best meet the needs of a particular
individual. All Cardium Health Senior Health Management Clinicians have extensive experience
in community health and counseling, which gives them the clinical knowledge and human
perspective to connect with individuals where they are. Our clinicians listen for the
story behind the facts and the numbers. For participants, the 30-minute assessment
call is an opportunity to get a sense of what the Cardium Health counseling program is
really like.
