DM Track is a specialized set of networked disease management applications that allow tracking, scheduling, documenting and reporting of the counseling process and participant progress.

Enabling technology for better health.   The Cardium Health real caring model and counseling process is enabled by DM TrackSM, a proprietary data management system. DM Track is a specialized set of networked disease management applications that allow tracking, scheduling, documenting and reporting of the counseling process and participant progress.

Cardium Health multi-disciplinary clinicians use one or more DM Track applications during each participant session. Information is organized by individual participant and includes:

  • Demographic and physiologic data
  • Lifestyle markers
  • Health improvement plans with goals and interventions
  • Physical activity indicators and goals
  • Diet and nutrition tracking
  • Medical history, medication details, scheduled appointments and results of lab tests
  • History of educational mailings and other communications related to program eligibility

Clinicians add information to DM Track when and where appropriate. They also are prompted to update selected fields, which are used for charting and reporting progress in aggregate form. Additionally, the DM Track system has built-in quality tools that help to assure that all calls and other forms of communication occur on-time and with accuracy.

Every element of data entered into and stored within the DM Track database is categorized as personal health information and is protected at all times against intrusion and disclosure. Cardium Health applies the best available security measures as required by federal regulations (HIPAA), which mandate the guaranteed assurance of privacy and security for personal health information.

An electronic medical record
From a health counselor’s perspective, DM Track is an electronic medical record that organizes and stores clinical data in chronological order, such as diagnosis, symptoms, labs (cholesterol, A1C, etc.), and weights. It also provides a medication tracking system for all prescriptions and dosages. DM Track links to a medication database, which provides both clinicians and participants with detailed information including:
  • Description of the medication
  • Precautions and side effects
  • Proper use

Participants are often wary of new medications. This real-time tool provides the detailed information that helps make participants feel more comfortable, which in turn encourages them to be compliant.

Tracking success
One of the most important aspects of DM Track is its ability to allow clinicians to follow participants on their journeys to better health. Improvements in physical activity such as levels of frequency, duration and type, changes in diet, weight and attitude, as well as clinical measures such as heart rate and lab values are all recorded. Whenever a participant has a consult with a Cardium Health exercise physiologist, the report is scanned into the document section and is available for the primary health coach to review and use to discuss ongoing progress with the participant.

DM Track lists many blood glucose monitoring devices (meters) for participants in the diabetes disease management program and provides direct links to the appropriate web sites for additional information, technical support and usage assistance over the phone. Often, common problems that people encounter with their meters are solved simply, which helps participants comply with regular blood glucose monitoring. In addition, information on insurance coverage and state laws mandating insurance coverage of diabetes testing supplies are listed.
