Customized enrollment with a blended opt-in / opt-out model. For
employers, the goal of wellness, lifestyle and disease management programs is to improve
productivity, reduce absenteeism, and control rising healthcare costs. But for individual
employees, the goal is much different. It’s personal. That’s why Cardium
Health takes a people-centered approach to connect with individuals wherever they are
in the healthcare continuum.
Many valuable programs are available to assist healthy individuals to stay healthy,
to help at-risk individuals improve their health, and to work with people experiencing
chronic conditions or serious illness. These programs have the potential to produce valuable
returns for benefit plan sponsors. However, they all are limited in their ability to
deliver real ROI by one common factor – the challenge of getting individuals to
participate.
A variety of models are used to meet the challenge of increasing participation. “Opt-in” approaches
allow participants to voluntarily enroll by calling or completing an electronic or paper
form. “Opt-out” approaches automatically enroll all identified program candidates
unless they take specific action to disenroll. Hybrid approaches combine elements of
both models depending on the status of the candidate or participant. In all these cases,
individuals are identified through a data analysis process called “predictive modeling.”
At Cardium Health, we do things a little differently. Our real
caring model goes beyond
predictive modeling. While respecting the personal privacy of members, we work to identify
potential candidates for our programs through multiple information sources including
health plan and pharmacy benefit claim data and disability data. We also work with
employers’ vendor-partners to generate data through health risk assessments and
wellness screenings. And we encourage referrals from vendor-partners, physicians, as
well as candidate self-referrals. By considering all of these sources of information,
we identify the largest number of potential candidates who may benefit from our programs.
Next, we do the hard part. We call them. Each and every person is contacted. If we don’t
have a working phone number, we’ll do everything possible to find it, from directory
assistance to Internet searches. Cardium Health Outreach Coordinators (OCs) talk with
candidates about the privacy of their healthcare information and reassure them that it
will only be shared between them and Cardium Health staff who receive special training
in privacy practices. The OC explains that our programs are free to participants, and
that we are there to help.
We then ask them to take the first step in what often becomes a life-changing journey.
The OC asks to schedule an assessment call with a Cardium Health Senior Health Management
Clinician. More than 60% of the people we talk to say yes. Sure, it’s an expensive
approach. Sure, it takes time and energy. Sure, it means we work nights and weekends
to connect with everyone. But it’s the only way we know how to do what we do.
